How it works
What you'll need
- Your current medication list
- Relevant medical history
- Your pharmacy's name and zip code
- Previous lab results (for refills)
Note on refills: For Bridge refills of chronic medications, Li Health may request recent lab work or records from your previous provider to ensure safe, continuous care. We make this process as smooth as possible.
Li Health is a focused virtual practice specializing in two areas: weight loss medicine and urgent care. This focused approach allows us to provide the highest quality care in both specialties rather than trying to be everything to everyone.
We do not provide ongoing management of chronic conditions such as hypertension, diabetes, thyroid disease, heart disease, or other long-term medical problems. For chronic condition management, continuity of care, or specialist referrals, please see your primary care physician.
One exception — short term medication bridge refills:
If you are an established patient and find yourself temporarily without access to your regular physician, we can provide a short term refill bridge for certain chronic condition medications to ensure you are not left without essential medications. This is a one time or limited courtesy — not a replacement for ongoing care with your primary doctor.
